This is the youth consent form for allowing your youths information on our media such as Facebook, Instagram, Website, and any other form of media.

Small field trip consent
This is the youth small field trip consent form. Once this is on file, your youth can go on any field trip within a 100-mile radius of Fairhope Christian Church. If there is a further than 100 miles trip, we will create a specific field trip consent form for that trip.
